As parent or legal guardian of the above named individual, for valuable consideration received, I hereby grant to the National Association of Drug Court Professionals and its legal representatives, licensees, and assigns the irrevocable and unrestricted right to use and publish in any manner and medium including editorial trade, advertising, and any other purpose deemed appropriate by NADCP--consistent with and in furtherance of NADCP’s mission, goals and purpose--photographs and videos of the individual; photographs and videos in which the individual may be included; any work of art, including any drawing, painting, illustration, poster or other original rendering created by the individual; and, to alter and edit without restriction the same for space or size considerations. On behalf of the named individual, I hereby release the National Association of Drug Court Professionals and its legal representatives, licensees, and assigns from all claims and liability relating to said photographs, videos or works of art.
